Bridging the Gap to Home

Our rehab team works with you or your loved one to reach health goals as quickly and efficiently as possible. We partner with physical, occupational and speech therapists to achieve these aspirations of independence. We provide equipment and highly trained staff to achieve the balance of making this possible. Windsor’s ultimate goal is to safely return residents to their homes, or other appropriate living arrangements, as soon as possible. While each resident’s therapy regimen is prescribed by their physicians, progress towards goals is assessed weekly in order to monitor the treatment plan to ensure clinical needs are met and/or exceeded. Once rehabilitation goals have been achieved, discharge planning occurs including home assessments and in-depth caregiver training to support our resident’s continued success in their own environment.

Physical Therapy

Physical therapy focuses on identification and rehabilitation of individuals with physical impairment and pain, as well as future injury prevention. Our physical therapy team’s goal is to develop a program that is based on individualized needs.

Occupational Therapy

Our occupational therapists work with residents in adapting to their social and physical environment by working on tasks essential for daily living. An occupational program might include working on functional independence for things such as cooking, eating, bathing, dressing and grooming. Our therapists also work with residents in increasing safety awareness for when residents return home.

Speech Therapy

Our speech and language therapists work with residents affected by stroke, brain injury, dementia or those suffering from hearing impairment, respiratory illnesses and other complex medical conditions. The speech therapy program is designed to improve speech, increase comprehension and verbal expression, improve chewing and swallowing and maximize cognitive skills.
